1. Covid-19 screening questions below: Please review and if you answer yes to any questions you are to call your attorney or the Court Clerk, Barbara Lloyd @ (914) 277-8225 You will not be admitted to the Courthouse.

Questions:

A)          Have you experienced any symptoms of COVID-19 (cough, shortness of breath or difficulty breathing, fever, chills, muscle pain, sore throat or new loss of taste or smell) in the past 14 days?

B)           Have you tested positive for COVID-19 in the past 14 days?

C)           Have you been in close contact with anyone who is a confirmed or suspected positive case of COVID-19 case in past 14 days?

D)          Have you traveled to any of the states on New York’s travel advisory list?

2. All Court visitors shall wear MASKS to enter the Courthouse

3. All Court visitors shall maintain a distance of at least six feet from other person whenever possible
